Farmakokinetik och kvantitativ farmakologi
Beskrivning
Vår forskning använder farmakometrisk metodologi där vi undersöker farmakokinetik och dess relation till önskade och oönskade effekter (farmakodynamik). Vi integrerar information från pre-kliniska experiment till kliniska studier för att facilitera translationell medicin, läkemedelsutveckling och strategier för dosindividualisering. Vi använder innovativa tekniker för att ta fram strategier för optimal läkemedelsanvändning och läkemedelsutveckling mha artificiell intelligens och maskinlärningsprocesser.

Translationell medicin
Att förutspå effekter i människan baserat på pre-klinisk information är avgörande i läkemedelsutveckling. Vi använder mekanistiska modeller i flera nivåer för att beskriva läkemedelseffekter på biomarkörer (se figur). Ett exempel är ”Multi-State Tuberculosis Pharmacometrics Model” (Clewe et al 2016 JAC) som framgångsrikt har tillämpats för att beskriva både in vitro och in vivo data, och har använts för att förutsäga behandlingsrespons i tuberkulos patienter (Wicha et al 2018 CPT, Susanto et al 2020 CPT).
Läkemedelsutveckling
Vi jobbar nära ledare av kliniska studier, infektionsläkare och läkemedelsbolag för att understödja design av kliniska studier och analysera data på mest effektivt sätt. Vi arbetar med metoder för att optimera studiedesign av kliniska prövningar (Svensson et al JAC 2017). Vi har också utvecklat innovativa mekanistiska modeller för biomarkörer som beskriver behandlingsrespons för tuberkulosläkemedel (exempelvis Svensson et al 2018 JID, Svensson et al 2019 AAC).
Individualiserad behandling
Det är mycket vanligt att tuberkulos patienter också bär på en HIV-infektion eller drabbas av malaria. Samtidig behandling av flera sjukdomar är därför nödvändigt men kan leda till läkemedelsinteraktioner. Interaktioner kan leda till minskad effektivitet eller ökad risk för biverkningar då individualiserad behandling bör övervägas. Vi har kvantifierad farmakokinetiska läkemedelsinteraktioner för ett stor antal läkemedelskombinationer. Interaktioner kan också ske inom farmakodynamiken och för att hantera detta har vi utvecklat ett innovativt analysverktyg kallat “The General Pharmacodynamic Interaction” (GPDI) modellen (Wicha et al 2017 Nature Comm). GPDI modellen kan hantera tids- och koncentrationsberoende interaktioner för synergi så väl som för antagonism med två eller flera läkemedel (Chen C 2018 AAPS J). Ännu ett forskningsfokus är optimal individualiserad behandling med dosanpassningar baserat på individuella mätningar av läkemedelskoncentrationer och/eller mikrobiologisk data och Bayesiansk inferens (Keutzer et al 2020 Front Pharmacol).
COVID-19 vaccinationsforskning
COVID-19, orsakad av coronavirus SARS-CoV-2, har resulterat i en pandemi med världsomspännande hälsoeffekter. Vaccination mot denna virusinfektion är avgörande för att förhindra sjuklighet och dödlighet. Vår grupp är en del av det EDCTP-finansierade Re-BCG-CoV-19 konsortiet som undersöker effekten av BCG-revaccination för att förhindra COVID-19 sjuklighet och dödlighet hos vårdpersonal i Sydafrika. I denna fas III-vaccinationsstudie inkluderar 1000 individer som arbetar inom sydafrikansk sjukvård. Shiny applikation för parametrisk time to eventanalys
Vi har utvecklat en innovativ Shiny-applikation för att vägleda den farmakometriska modelleraren genom utvecklingsprocessen av en parametrisk time to eventmodell. Parametrisk time to eventanalys är ett viktigt verktyg i farmakometrikerns verktygslåda för att analysera händelsedata. Exempel på händelsedata är förekomsten av en luftvägsinfektion så som COVID-19, en myokardinfarkt eller tid till positivt i en vätskebaserad odlingsanalys för tuberkulos. Sannolikheten för att en händelse inträffar vid en viss tidpunkt kan kvantifieras som en funktion av riskfaktorer, inklusive läkemedelsexponering, med denna fullständiga parametriska metoden.
